Creative Home Services, Inc. is a nurse owned and operated home care agency. We have provided home health care and ancillary services to the residents of Cape Cod for 9 years. CHS services are privately paid and are based upon a care management model. Our speciality is advocating for the elderly.

What We Do…

Our goal is to provide our client's with an individualized, effective care plan. The CHS care management team is client focused and knowledgeable in community resources. We will assess the client's overall situation and develop specific goals. Our registered nurses will develop a plan of care that will provide appropriate care options for client and family. CHS services are flexible and dynamic, ready to adapt to changing demands.

From skilled nursing care to in-home companionship, CHS provides a comprehensive menu of in-home and interagency services. All levels of elder support services are available. Creative Home Services, Inc. will care for clients in their private homes, as well as work in concert with long-term care facilites, assisted living programs, and hospitals to provide one-to-one care when needed. In addition, we can provide placement assistance if needed.

When…

The inital step is to call CHS to schedule a meeting. Our nurse will assess the prospective client as well as any family needs or concerns. As our client, you will have the security of our on-call Registered Nurse to address issues that may arise at any time, day or night.
