The Care Manager draws upon the practice of medicine, community health, psychology, nutrition and sociology in making their decisions. COORDINATION The Care Manager develops and coordinates a plan of care. The Care Manager acts as liaison to community resources. The services coordinated may include: transportation, meal prep, housekeeping, nursing, personal care aides, social services, and outings to special events. The Care Manager may also oversee privately paid caregivers or arrange for one-to-one care givers as needed. The Care Manager works closely with the client, family, physicians, facilities and community services. COST Care and cost are complimentary concerns. When appropriate, clients are referred to Medicare reimbursable services. The Care Manager works in partnership with the family and the client to present a safe and cost effective plan. The goal of care is to have the most effective lifestyle support services, allowing the client the absolute highest level of independence.
